Mental Health Leave Assessment (MHLA)
An evaluation of reported mental health concerns, clinical findings, and their effect on work-related functioning. The assessment may inform documentation for a request for continuous or intermittent leave when clinically supported.
FMLA documentation
Bring the employer’s certification form, job description, essential duties, and submission deadline. The evaluator reviews the clinical basis for leave, relevant functional limitations, and estimated duration or frequency when supported by the information available.
FMLA eligibility and leave designation are determined by the employer or leave administrator. An assessment appointment does not guarantee certification or approval.

Reasonable Accommodation Assessment
An evaluation focused on how mental health concerns affect essential work activities and what adjustments may support functioning.
Areas reviewed may include concentration, task completion, emotional regulation, communication, social interaction, and environmental demands. Potential recommendations depend on the individual assessment and actual job requirements.
Bring the employer’s reasonable accommodation form, essential job functions, and any specific request for documentation. FMLA and reasonable accommodation serve different purposes and may require different forms.
The evaluator provides clinical findings and recommendations when supported. The employer determines accommodations through its interactive process; a specific arrangement, including telework, is not guaranteed.

Parenting Capacity Assessment
A structured evaluation of parenting strengths, support needs, and factors relevant to a parent’s ability to meet a child’s needs. The scope is agreed before services begin and is guided by the referral question and the child’s developmental context.
Possible areas of review
- Caregiving history, routines, and understanding of the child’s needs.
- Parenting skills, emotional responsiveness, and protective supports.
- Stressors, family resources, and areas where additional support may help.
- Relevant records, interviews, and observations when appropriate and authorized.
Before scheduling an evaluation
Identify who requested the assessment, its intended use, who will receive the report, and any deadline or court order. PEOLLC must review evaluator qualifications, referral fit, conflicts of interest, and jurisdiction requirements before accepting a referral.
This service is not automatically a custody evaluation, forensic psychological evaluation, or expert-witness service. Court-related requests require scope review before acceptance. No custody or court outcome is promised.
What to expect
- Consultation: Select an available time through Zoom Scheduler and identify the service requested. Booking is a consultation request, not acceptance of the evaluation referral.
- Scope & consent: Confirm the purpose, evaluator role, fees, document needs, confidentiality limits, and authorized report recipients.
- Evaluation: Complete the agreed interviews, record review, and appropriate assessment activities. Additional information or appointments may be needed.
- Findings & next steps: Receive an explanation of findings and any agreed documentation that the evaluation supports. Recommendations may be deferred if evidence is insufficient.
Fees, payment arrangements, service location, and turnaround time are confirmed before evaluation services begin. Availability depends on referral fit and applicable professional licensure.
Prepare for your consultation
Have your referral request, required forms, deadlines, job description for work-related requests, and a list of relevant providers or records available. For parenting referrals, have the referral questions and any applicable orders ready for review.
Use the scheduler for appointment details only. Do not enter medical histories, employer documents, child information, or dispute details in booking notes or general email. Our team will explain how to provide records through the appropriate private process.
